Care to Change
Care to Change is a Registered NDIS Provider supporting people across South East Queensland to build independence, develop skills, and stay connected to their community.
Our Morayfield Hub is a welcoming, inclusive space where people can come together to connect, learn new skills, and take part in meaningful activities. From social groups and community outings to structured skill-building programs, the Hub is designed to support participation, confidence, and everyday independence.
We are registered to provide a wide range of NDIS supports, including:
- Assistance with daily personal activities, including high intensity supports
- Supported Independent Living and daily tasks in shared living
- Community participation and group or centre-based activities
- Support coordination and assistance with life stage transitions
- Development of life skills and capacity building supports
- Travel and transport assistance
- Community nursing care and behaviour support
At Care to Change, we are committed to improving care for everyone by delivering flexible, person-centred supports that respond to each individual’s needs, while creating a safe and engaging environment at the Morayfield Hub where people can grow, connect, and achieve their goals.
